GPWS “Terrain Ahead” Serious Incident Involving SalamAir Flight OV104

While conducting a visual approach to Muscat International Airport on 30 September 2024, the crew of SalamAir Flight OV‑104/A4O‑OXG received a “Terrain Ahead” warning from the EGPWS/GPWS. They initiated a climb, avoided terrain, and continued the approach safely. No injuries or damage were reported. The incident has been classified as a serious incident.

Flight Details

  • SalamAir Airbus A321-NEO, registration A4O‑OXG, was inbound from Salalah (OV‑104) to Muscat (OOMS) on 30 September 2024. 

Phase of Flight & Incident

  • During visual approach at approximately 5,000 ft MSL, crew received a “Terrain Ahead” caution from the Enhanced Ground Proximity Warning System (EGPWS). 

Crew Response & Action

  • Cargo altitude was levelled, then climbed to 8,000 ft. Crew then repositioned heading to 355° to maintain safe terrain clearance and continued with the approach after stabilization. 

Outcome

  • The flight landed safely on Runway 08L after requesting vectors for ILS final approach. No injuries or aircraft damage occurred. 

Classification & Investigation

  • Rated a serious incident by OTSB. Investigation aligns with ICAO standards, aiming to improve safety via procedural and training recommendations.
